If you have a chronic health condition, your GP can refer you for up to 5 Medicare-rebated dietitian sessions per year. All via Zoom, anywhere in Australia.

  • Type 2 diabetes or pre-diabetes
  • High cholesterol or cardiovascular disease
  • Obesity or weight-related health concerns
  • GLP-1 medication support (Ozempic, Wegovy, Mounjaro)
  • IBS, coeliac disease, or gut conditions
  • PCOS, kidney disease, or hypertension
  • Malnutrition or unintentional weight loss

How to get started
  • See your GP and ask about a GP Chronic Condition Management Plan (GPCCMP). Request a referral to CCDietitian.
  • Book your appointment below. We will confirm receipt of your referral and get you scheduled.
  • Your initial Zoom consultation. Convenient, at a time that suits you.
