Medicare Rebates Available
Medicare & GP Referral Patients
Dietitian support for Medicare-referred patients, working alongside your GP.
If your GP has identified that you'd benefit from dietitian support, you may be eligible to claim a Medicare rebate through a Chronic Disease Management (CDM) plan, formerly known as an Enhanced Primary Care (EPC) plan. This allows you to access up to 5 dietitian sessions per calendar year, with a rebate paid back to you after each visit.
Am I Eligible?
  • You have a chronic medical condition (e.g. diabetes, heart disease, kidney disease, obesity)
  • Your GP has prepared a GP Management Plan (GPMP) and Team Care Arrangement (TCA)
  • You have a valid referral from your GP for dietitian services
  • You have remaining sessions available within the current calendar year
How the Fees & Rebate Work
Unlike bulk-billed services, you pay the full consultation fee at the time of your appointment, and a Medicare rebate is then paid back to you separately (either on the spot or through your Medicare online account). A gap remains payable each visit, but under a GP Management Plan you can claim the rebate for up to 5 sessions per calendar year.

Appointment Structure Under Your GP Plan
We start by getting to know you — your health history, current eating habits, lifestyle, and the goals set out in your GP plan — and build you a personalised roadmap, complete with recipes and practical tools that fit into your life. From there, we continue meeting to review your progress, fine-tune your plan, and work through any challenges, all the way until you reach your goals.
Your GP plan covers 5 sessions per calendar year, each with a Medicare rebate — but there's no need to stop there. If you and your GP feel it's helping, your GP can renew your plan for a further 5 rebated sessions the following year, and we're always happy to keep supporting you beyond that too.
